• This instruction applies to all EASA CAT A staff performing Independent Inspections of Critical Tasks under RAS Technic procedures.
• To ensure compliance with safety management system (SMS) requirements, this procedure provides guidelines for EASA Part 145 Independent Inspectors (CAT A) to report safety concerns, hazards, or incidents encountered during Critical Tasks inspections.
When to Report:
• Incorrect completion or omissions during Critical Tasks.
• Potential hazards or unsafe conditions identified during inspections.
• Non-compliance with maintenance data or procedures.
• Near misses, discrepancies, or defects not addressed correctly.
How to Report:
>> Use the Part 145 Safety Reporting Form or designated electronic platform.
>> Include detailed descriptions:
• Date, time, and location.
• Aircraft registration and task reference.
• Description of the identified hazard, event, or failure.
• Actions taken to contain the issue.
>> Submit the completed form to the Safety & Compliance Monitoring Manager (SCMM) within 24 hours.
Confidentiality:
>> All safety reports will be treated confidentially under the SMS guidelines. No disciplinary actions will occur for genuine reporting unless gross negligence is identified.
Follow-Up:
>> The Safety & Compliance Department will assess the report, investigate the cause, and provide feedback within 7 working days.
>> Corrective actions will be implemented to prevent re-occurrence, and lessons learned will be disseminated to the team.
Responsibilities

>> Diligently identify and report safety-related concerns or non-compliance.
>> Complete all reports accurately and promptly.
• SCMM:
>> Oversee the reporting process and ensure timely resolution.
>> Maintain safety reporting records and monitor trends for SMS performance.
